From The Sims Medieval Wiki
The Physician is one of the playable heroes in The Sims Medieval. They are responsible for keeping your villagers free of illness and injury.
Your physician will be available to you when you acquire the clinic.
You will make a daily income by treating patients. You can also craft your own recipes and sell them at the Village Shop. Gather those herbs and wildflowers on the way to the shop as they sell as well!
It is up to your physician to keep your Kingdom safe and sound. They are responsible for treating illness and injuries that will befall your villagers. Luckily for your Kingdom, your physician has certain abilities that help him or her to be prepared for any catastrophe. Your physician can perform surgical procedures such as leeching. They can also craft medical potions to treat many ailments.
Of course, this just does not happen with a snap of your fingers. Alas, you must perform certain tasks to keep your Kingdom in tip top shape.

You must first gather your herbs and such to create potions. Bloodletting requires leeches. Lucky for you, your physician can collect most of their items as they travel out and about. In a pinch, you can find many of the ingredients at the village shop.
You can find many of the plants needed for your potions in the clinic garden, (located in your very own front yard). No need to water your plants. Just discover the wonders of your garden and then pick the plants. You must first click the plant to discover it before you can collect the ingredient.
Your physician may even get a nice focus buff when he or she first discovers a plant!
You can find many herbs and roots by just exploring your kingdom. There are many plants found along the path from the clinic to the town square.
Did someone say Leeches?
Indeed, you will need to gather those slimy leeches before you can perform certain medical procedures. Head out by the docks and look for those green bubbles. You can also find many leeches in the river that winds through your Kingdom. Why are Leech bubbles green, you ask? Its probably best not to know the answer.
Potions and Remedies
Now that you have gathered your items, it is time to get down to actually performing your craft. Head into your clinic and click on your crafting table to see what recipes you can prepare.
Once you have created your potions, you can either sell them for a small profit or use them to treat patients. You also gain xp each time you successfully craft a potion. Gaining experience through crafting helps you to further your level progression, which in turn, allows you to craft better recipes.
Surgical and Medical Procedures
You physician can perform bloodletting to "cleanse" your patient of illness. Ever heard the term bad blood? Well, now you know where is comes from.
Ask your patient to lie on the operating table by clicking on your table and choosing your patient. Once he or she is comfy you can click the table again to choose the bleeding option.
Now lets uses those leeches you found earlier. Click your patient and choose how many leeches you wish to apply. Before you accidentally bleed them dry, you may want to take a look at your blood meter (upper right corner of your hud). You only want to apply enough leeches to get the bad blood level reduced. Do this by applying one leech at a time until your patient's blood level hits the yellow mark.
